When envisioning a patient with type 2 diabetes, it is common to picture someone who is overweight. This assumption is supported by the statistic that approximately nine out of ten individuals with type 2 diabetes in developed nations are classified as obese or overweight. However, this narrative shifts significantly in regions such as Africa, where approximately four out of ten adults diagnosed with type 2 diabetes are of a normal weight. In specific countries, like Ghana, this figure rises to six out of ten. Sabrina Esmail, a researcher from Amsterdam UMC, emphasises that this indicates an estimated 10 million individuals in Africa who do not conform to the conventional profile of diabetes patients.

It is widely acknowledged that type 2 diabetes can affect both overweight and slender individuals, but the implications of this distinction are profound. While both groups suffer from the same disease and typically receive similar treatments, emerging research reveals that the underlying biological mechanisms differ considerably. In overweight patients, type 2 diabetes primarily arises from insulin resistance, whereas in those who are thinner, the condition is often linked to pancreatic insufficiency, resulting in inadequate insulin production. Esmail notes that for this latter group, the critical issue lies in insulin deficiency rather than a diminished response to the hormone.

This significant divergence suggests that millions of individuals with type 2 diabetes may be receiving inappropriate treatment strategies.

The Distinct Pathways of Diabetes

Sabrina Esmail is part of a collaborative research team from Amsterdam UMC and the University of Ghana, which examined data from 3,331 individuals across Ghana, Nigeria, and Kenya. Their findings indicate that the progression of diabetes can vary significantly based on the patients’ body mass index (BMI).

The study elucidates notable differences in complications associated with diabetes between the two groups. For instance, slender patients tend to experience a higher incidence of ocular damage, known as retinopathy, as well as strokes. Conversely, overweight individuals are more frequently afflicted by hypertension and have an elevated risk of cardiovascular diseases. Interestingly, chronic kidney disease was observed to occur with similar frequency in both cohorts, according to Felix Chilunga, the lead author of the study.

The variation in body fat distribution and overall quantity contributes substantially to these differences, but there are also contrasts in risk factors. While overweight individuals are often linked to unhealthy lifestyle choices, thinner patients frequently have a history of malnutrition or low birth weight, which can adversely affect pancreatic development.

The Insulin Factor in Treatment Approaches

The study places particular emphasis on insulin levels. Analysis of participants revealed that slender individuals with diabetes who were not on insulin therapy exhibited markedly lower concentrations of the hormone and significantly impaired function of pancreatic beta cells, which are responsible for its production. These findings support the hypothesis that, for this demographic, the primary issue may be insufficient insulin secretion rather than insulin resistance.

Common treatment strategies for type 2 diabetes in many African countries are frequently guided by international protocols advocating medications such as metformin, regardless of whether patients have a low or high BMI. While this approach effectively reduces insulin resistance, it may not adequately address the needs of those whose primary challenge is low insulin production. Consequently, these patients may be receiving suboptimal treatments, leading to poor glucose control and a heightened risk of complications over time.

Recognising Limitations and Future Directions

The authors of the study caution that their findings do not definitively establish a cause-and-effect relationship between the type of diabetes and the observed differences. Being a cross-sectional analysis, it does not allow for causal inferences. They also noted a lack of detailed data on factors such as access to healthcare, medication adherence, and frequency of medical consultations. Therefore, they advocate for longitudinal studies and clinical trials to identify the most appropriate diagnostic and treatment strategies.

Despite these limitations, the consistency of the results among participants reinforces the notion that type 2 diabetes in slender individuals may represent a distinct phenotype of the disease. In Africa, where this profile is more prevalent, approximately 24 million adults live with type 2 diabetes, with nearly two-fifths being of a normal weight. Furthermore, this issue is not confined to the African continent; a significant number of African patients residing in Europe also present with lower BMIs yet are treated according to guidelines designed for overweight individuals, resulting in poorer disease management, as noted by Charles Agyemang from Amsterdam UMC.

The Urgent Need for Tailored Treatment Strategies

Understanding these differences is crucial to ensuring that patients receive treatments that align more closely with the biological realities of their condition. This study marks a vital step in highlighting the urgent need for improved treatment options for the considerable group of slender Africans facing diabetes. Agyemang articulates a significant concern, stating that the complexities of type 2 diabetes, a life-threatening condition affecting millions globally, remain inadequately explored.

Felix Chilunga calls upon global health authorities to initiate specific clinical trials to determine the most effective treatments for this often-overlooked patient demographic. The research findings were published this week in the academic journal Diabetologia, shedding light on a pressing health issue that warrants immediate attention.
